During the "review your order" portion, in the upper section just beneath your "shipping address" and the line which reads "shipping method," there will be a line which reads "desired inscription (optional) or 'none'" and a link which says "Add" -- following that link will open an optional dialog box that enables you to enter a message you would like inscribed inside the book.
